Recipe 11.5: Python Implementation Example
Heads up: This is a deliberately simple, illustrative implementation of the pseudocode walkthrough from Recipe 11.5. It shows one way you could translate the insurance-benefits-navigator pipeline into working Python using boto3 against Amazon Bedrock (LLM with function-calling), Amazon Bedrock Knowledge Bases (managed RAG over the per-plan-and-year plan-document corpus), Amazon Bedrock Guardrails, AWS Lambda, Amazon API Gateway, Amazon DynamoDB, Amazon S3, and Amazon EventBridge. The demo uses a

It is not production-ready. There is no real Bedrock Agents action group configured, no real Knowledge Base ingestion, no real Guardrail configuration, no API Gateway plumbing, no WAF rule tuning, no per-Lambda IAM least privilege, no KMS customer-managed keys, no VPC endpoints to the eligibility, claims, accumulator, UM, formulary, or provider-network systems, no Object-Lock-protected decision-record journal, no Connect contact-center handoff, no CTI integration with member-services, and no Secrets Manager wiring for the upstream-system credentials. Think of it as the sketchpad version: useful for understanding the shape of a benefits-navigator AI pipeline that respects the input-screening discipline, the per-plan-and-year plan-document-corpus discipline, the citation-grounding discipline, the deterministic-cost-estimate discipline, the regulatory-disclosure discipline, the member-services-routing discipline, and the audit-everything discipline this recipe demands. It is not something you would point at a payer's member portal on Monday morning. Consider it a starting point, not a destination.The code maps to the ten pseudocode steps from the main recipe: receive the message, bootstrap or resume the session, run input safety screening with benefits-specific crisis-and-financial-distress detection (Step 1); load plan context, accumulator state, and subscriber context (Step 2); classify intent and route to the right tool surface (Step 3); handle coverage questions with strict version-scoped plan-document retrieval (Step 4); handle network-status questions with rendering-provider-vs-facility distinction (Step 5); handle claim-explanation questions with CARC/RARC translation (Step 6); handle cost-estimate questions through a deterministic compute tool (Step 7); run output safety screening with citation verification, plan-version stamp consistency, and regulatory-disclosure presence checks (Step 8); persist the durable benefits-decision record alongside the conversation log (Step 9); close the conversation, archive the audit record, and emit per-cohort metrics (Step 10). The synthetic members, plans, claims, providers, accumulator state, and decisions in the demo are fictional; nothing in this file should be interpreted as advice from any real payer.
Setup

In production you would also configure an Amazon Bedrock Agent (or a custom Lambda-based orchestrator) with action groups defining the benefits tools (plan_context_lookup, accumulator_lookup, subscriber_context_lookup, intent_classify, plan_document_retrieval, coverage_lookup, provider_network_lookup, claim_lookup, carc_rarc_translation, prior_auth_lookup, formulary_lookup, cost_estimate_compute, aeob_or_gfe_lookup, member_services_route), each backed by a tool-implementation Lambda that wraps the payer's eligibility, claims, accumulator, utilization-management, formulary, and provider-network systems. You would also configure an Amazon Bedrock Knowledge Base ingesting curated content from S3 covering the per-plan-and-year plan-document corpus (Summary of Benefits and Coverage, Evidence of Coverage, Schedule of Benefits, member handbook), the persona-and-tone guidance, the regulatory-disclosure-phrasings library, and the closing-template library, with metadata-filtered retrieval scoped to the member's plan and plan year. You would configure an Amazon Bedrock Guardrail with restricted-topic filters for clinical-advice, off-label-drug-recommendation, binding-coverage-commitment, and diagnostic-speculation categories at minimum, an API Gateway endpoint fronting the chat-handler Lambda, an AWS WAF web ACL with rate limits tuned for the multi-question benefits-navigation pattern, the five DynamoDB tables (conversation-state, conversation-metadata, tool-call-ledger, benefits-decision-record-journal, version-stamp-registry), an Amazon S3 bucket with Object Lock in compliance mode for the benefits-decision-record journal sized to the longest of HIPAA's six-year minimum, the state's insurance-record-retention rules, the state-specific consumer-financial-information retention rules where applicable, and the institutional regulatory floor, an EventBridge bus for benefits-lifecycle events (conversation_started, intent_classified, retrieval_completed, answer_delivered, handoff_to_member_services, complaint_filed, conversation_closed), a Kinesis Data Firehose delivery stream for the conversation-audit archive, AWS Secrets Manager secrets for the eligibility-system, claims-system, accumulator-system, UM-system, formulary-system, and provider-directory credentials, and (where applicable) the Connect contact-center integration for the live member-services-agent handoff path. The demo replaces all of these with small mocks so the focus stays on the per-turn intent-classification, member-context-loading, retrieval-and-tool-orchestration, citation-grounding, cost-estimate-computation, output-screening, and decision-record-persistence logic rather than on the platform plumbing.

Scope each Lambda's IAM role to the specific resource ARNs it touches. The tutorial-level permissions above are fine for learning and will fail any serious IAM review. The chat-handler Lambda has Bedrock invocation and read-write on the conversation tables, but no direct access to the upstream benefits systems. The claim-lookup Lambda has read-only access to claims; it does not have permission to modify adjudicated claims. The cost-estimate-compute Lambda has read access to the cost-share-rule registry and the negotiated-rate database. None of the bot's Lambdas have write access to coverage decisions, claim adjudication, or member benefits records; the bot is read-only by design. Separation of concerns by Lambda role limits the blast radius of any single Lambda's compromise. Avoid wildcard actions and resources in production.
A few things worth knowing upfront:
- The plan-document corpus is the bot. The code below assumes the payer's per-plan-and-year plan documents (Summary of Benefits and Coverage, Evidence of Coverage, Schedule of Benefits, member handbook, formulary index) are curated, dated, version-controlled, and chunked with metadata for plan_id, plan_year, document_type, document_version, section_id, and effective_date. Cleaning up the corpus is the project. Skip the cleanup and the bot answers questions from the LLM's parametric memory rather than from the specific plan document, which is exactly the failure mode the architecture exists to prevent. The
PLAN_DOCUMENT_CORPUSplaceholder in the config is where you wire in your real corpus (in production this is the Knowledge Base index, not a Python dict). - Citation grounding is non-negotiable. Every coverage assertion, every cost number, and every regulatory-rights statement in the bot's response cites the retrieval evidence or the tool output that supports it. The output safety screening verifies the grounding before delivery. The audit record preserves the citation trail. Skip the grounding and the bot produces ungrounded answers that look authoritative, which is exactly what gets payers sued.
- Cost estimates run as deterministic code, not LLM math. The cost-estimate-compute tool encapsulates the deductible-then-coinsurance arithmetic, the embedded-vs-aggregate family logic, the separate-medical-and-pharmacy-accumulator handling, and the network-tier cost-share rules. The LLM phrases the result; the LLM does not compute the math. The demo's
cost_estimate_compute_toolfollows this pattern. - Regulatory disclosures are required phrasings, not optional warmth. Federal No Surprises Act protections, state insurance-complaint rights, parity-law disclosures for behavioral-health questions, appeal-rights disclosures for denial-explanation questions, and similar required phrasings are added to responses based on intent, plan type, line of business, and member state. The demo's
REGULATORY_DISCLOSURE_LIBRARYis illustrative; production has compliance-team-validated phrasings per state plus federal coverage. - Plan-version stamping is the audit floor. Every benefits-decision-record stamps the active plan-document version, the active formulary version, the active provider-network snapshot, the active cost-share-rule version, the active model ID, the active prompt version, and the active agent version. When a member disputes an answer ("you told me this was covered"), the audit record reproduces exactly which document, which version, and which member state produced the answer.
- The bot does not commit the payer to a coverage decision. The bot's answers are informational, grounded in the current plan documents and the current eligibility-and-claims state, and the actual claim adjudication remains the payer's claims-system process. The system prompt and the output-screening filters enforce this scope; binding-coverage-commitment language is replaced with the safer informational template.
- Conversation logs are dense PHI plus financial information. Members ask about specific medications, specific diagnoses inferable from procedures, specific bills, specific balances. The audit, retention, and access-control story matches HIPAA's PHI rules plus state-specific consumer-financial-information rules. The demo writes a redacted record; production writes through Firehose into an Object-Lock S3 bucket sized to the longest of HIPAA's six-year minimum, the state's insurance-record-retention rules, the state-specific consumer-financial-information retention rules where applicable, and the institutional regulatory floor.
- DynamoDB rejects Python
float. Every confidence score, threshold, deductible-met fraction, cost-estimate range bound, accumulator value, and numeric metadata field passes throughDecimalon its way in and on its way out. The_to_decimalhelper handles it. - The example collapses many Lambdas into a single Python file. In production the chat handler, the input-screening function, the identity-and-family-verification function, each tool-implementation function, the output-screening function, the decision-record-persistence function, and the audit-archival function are separate Lambdas with their own IAM roles, error handling, retries, and DLQs. Comments call out where the boundaries should fall.
Configuration and Constants
Everything that is configuration rather than logic lives here. Resource names, model IDs, the per-plan plan-document corpus, the cost-share-rule registry, the regulatory-disclosure library, and the validation thresholds are what you would change between environments.

The Gap Between This and Production
This demo runs end-to-end and produces the right disposition records, but the distance between it and a real benefits navigator serving a payer's member population is significant. Here is where that distance lives.
Real Bedrock Agents action groups (or a custom LLM-and-tool orchestrator). The demo runs a fixed Python flow that bypasses the LLM's tool-calling abilities entirely; the routing logic is hard-coded in the _classify_and_route chain and the response composition is template-based. Production wires the benefits tools as Bedrock Agents action groups (one OpenAPI spec per tool with the argument schema, the response schema, and the docstring the LLM uses to decide when to call it), gives the agent the institutional persona prompt and the plan-document Knowledge Base, and lets the LLM drive the multi-step reasoning, the tool-call orchestration, and the final response generation. The Python flow above is helpful for understanding what tools exist and what each one does; the production system lets the LLM compose the answer while the citation-grounding verifier keeps the structure honest.
Real Bedrock Knowledge Base ingestion of the plan-document corpus. The demo's PLAN_DOCUMENT_CORPUS is a hand-curated three-chunk dictionary. Production has a Knowledge Base ingesting curated content from S3 covering the per-plan-and-year SBC, EOC, Schedule of Benefits, member handbook, and formulary documents, with each chunk tagged with plan_id, plan_year, document_type, document_version, section_id, and effective_date. The retrieval query enforces plan-and-year scoping at query time. The corpus has a named owner (typically the benefits-administration team plus compliance plus the medical-director for clinical-policy aspects), a documented review cadence (typically per-plan-year refresh plus mid-year for amendments), and a versioned change-management workflow with sandbox testing against held-out benefits questions before each plan-document version goes live. Stale retrieval (the bot citing the prior plan year's document for a current-year question) is a serious failure mode the corpus governance prevents.
Real Bedrock Guardrails configuration. The demo passes GUARDRAIL_ID but does not actually configure a Guardrail. The Guardrail is pinned to a specific version, tested against a held-out evaluation set including benefits-injection cases (manipulate coverage-lookup to return unauthorized coverage assertion, manipulate cost-estimate to return zero cost, manipulate claim-lookup to return false denial reasons), and updated on a versioned-rollout cadence with canary traffic.
Real plan-document corpus governance with full version control. The single largest pre-deployment investment is getting the plan-document corpus correct, complete, and versioned. Each plan-year combination has multiple documents with effective dates, sometimes mid-year amendments, and re-issuance triggers. Most payers discover, partway through the project, that their plan documents are slightly inconsistent across formats (the SBC says one thing, the EOC says another in different language, the formulary index implies a third), that their accumulator system has subtle edge cases, and that their provider-network data does not surface the rendering-vs-facility distinction. Formalizing the corpus is multi-quarter work, and it is the highest-leverage investment the project will make. The plan-document corpus formalization typically takes three to six months of focused benefits-administration plus compliance work before the engineering project starts; skipping it is the most common reason benefits-navigator deployments fail in pilot.
Real cost-share-rule registry as code. The demo's COST_SHARE_RULE_REGISTRY covers one plan with simple rules. Production has the cost-share rules encoded as a structured registry that the cost-estimate tool reads, with per-plan-year version, change-management process owned jointly by the benefits-administration team and compliance, and stamped on every benefits-decision-record. The rules cover the full plan-type variation: HMO, PPO, EPO, POS, HDHP; embedded vs aggregate family deductibles; separate medical and pharmacy accumulators; copay-after-deductible vs coinsurance-after-deductible patterns; separate accumulators for in-network vs out-of-network; separate accumulators for preventive vs non-preventive; employer-specific plan customizations on the same base plan; mid-year plan-document amendments. The cost-estimate tool reads from this registry to produce arithmetic estimates that match what claims-system adjudication will produce for the same service.
Real CARC/RARC translation library with full code coverage. The demo's CARC_RARC_LIBRARY includes five codes. Production has the full set of CARC and RARC codes (hundreds of them) translated into plain English with next-step guidance, owned by the appeals-and-grievances team, reviewed annually and after CARC/RARC code-set updates. Each translation has a flag for "this is the kind of denial we typically appeal" versus "this is the kind of denial we typically don't" that drives the bot's offer-to-help-with-appeal logic.
Real regulatory-disclosure-phrasings library with state-specific configurations. The demo's REGULATORY_DISCLOSURE_LIBRARY covers federal No Surprises Act, parity, appeal rights, and cost-estimate caveats with single phrasings. Production has compliance-team-validated phrasings keyed by intent, plan type, line of business, and member state, with quarterly review cadence and explicit ownership. State-specific phrasings cover the state-by-state variation in insurance-complaint procedures, state-specific consumer-protection requirements, state-specific Medicaid managed-care rules where applicable, and state-specific prompt-pay or surprise-bill protections. The library is treated as a versioned governance artifact, owned by compliance.
Real eligibility, claims, accumulator, UM, formulary, and provider-network integrations through hardened tool wrappers. The demo's mocks are in-memory dictionaries; production has hardened wrappers around the payer's actual systems (FHIR-based for payers running FHIR-native infrastructure, vendor-specific APIs for older systems, an integration-engine layer for institutions that route everything through Mirth, Rhapsody, or Cloverleaf). Each wrapper handles every documented error code, retries idempotently with backoff, surfaces meaningful error categories to the bot rather than raw HTTP statuses, and is owned and maintained by the integration team. Plan multiple sprints for each integration; the LLM work is comparatively easy.
Real benefits-data-source freshness SLA per data class. Eligibility, claims, accumulator, prior-auth, formulary, and provider-network data each refresh on different cadences with different freshness expectations. Document the SLA per source. Telemetry alerts when a source's freshness exceeds the SLA. The bot's responses include as-of date stamps that come from the source-of-truth, not from the bot's invocation time. The bot's tools include the as-of date in every response, and the bot's answers are explicit about freshness ("based on data through April 8, 2026").
Real citation-grounding verification. The demo's citation check is a heuristic that just checks whether citations exist for member-specific intents. Production runs an independent verifier model with structured-output schema validation between Bedrock generation and response delivery, grounding every coverage assertion to a retrieved plan-document chunk, every member-specific claim to a tool result, every cost number to a cost-estimate-tool call, every regulatory-disclosure to the disclosure library. The faithfulness check uses rule-based contradiction detection, omission detection, a regenerate-attempt budget, and a fall-back-to-safe-response default. Per-cohort faithfulness-failure rate is a launch-gate metric.
Real DynamoDB and S3 wiring. The mocks are dictionaries; production is DynamoDB tables with on-demand or provisioned capacity, customer-managed KMS keys, point-in-time recovery on the conversation, ledger, decision-record, and version-stamp-registry tables, TTL on the conversation-state table tuned for typical session durations, and DynamoDB Streams emitting change events for downstream consumers. The benefits-decision-record-journal S3 bucket has SSE-KMS, Object Lock in compliance mode, lifecycle to S3 Glacier Instant Retrieval after 30 days and Glacier Deep Archive after 90, and retention sized to the longer of HIPAA's six-year minimum, the state's insurance-record-retention rules, the state-specific consumer-financial-information retention rules where applicable, and the institutional regulatory floor. The audit archive has its own KMS key separate from the decision-journal KMS key for blast-radius containment.
KMS customer-managed keys per data class. Every PHI-bearing or PFI-bearing resource uses customer-managed KMS keys with key rotation enabled. Different KMS keys for different data classes (conversation-state vs decision-journal vs audit-archive vs Secrets Manager secrets) limit the blast radius of any single key compromise. Key access is scoped to the specific principals that need it; CloudTrail logs every key-usage event.
VPC and VPC endpoints. The chat-handler Lambda runs internet-facing through API Gateway, which is the public design. The tool Lambdas that call the eligibility, claims, accumulator, UM, formulary, and provider-network systems run in a VPC with PrivateLink (where supported) or a tightly-scoped NAT-gateway path with allow-list. VPC endpoints for Bedrock, DynamoDB, S3, KMS, Secrets Manager, EventBridge, and CloudWatch Logs keep AWS-internal traffic off the public internet.
WAF tuning for the multi-question benefits-navigation pattern. Benefits endpoints have moderate rate limits because legitimate members sometimes ask many short questions in a row when navigating a bill. WAF rules also apply bot-detection rules that allow legitimate accessibility tools while blocking automated abuse, plus geo-restrictions if applicable, plus managed rule groups for common attack patterns.
Per-Lambda IAM least privilege with separation of concerns. The demo uses a single set of mocked credentials. Production has one IAM role per Lambda (chat handler, input screening, identity handling, each tool implementation, output screening, decision-record-persistence, audit archival), each scoped to the specific resource ARNs the Lambda touches. The claim-lookup Lambda has read-only access to claims; it does not have permission to modify adjudicated claims. The cost-estimate-compute Lambda has read access to the cost-share-rule registry and the negotiated-rate database. None of the bot's Lambdas have write access to coverage decisions, claim adjudication, or member benefits records; the bot is read-only by design.
Per-cohort accuracy and equity monitoring with launch gates. The demo emits CloudWatch metrics with per-channel and per-language dimensions, which is enough for per-category dashboards. Production stratifies by cohort axes the institution monitors (per-language, per-channel, per-line-of-business, per-plan-type, per-age-cohort, per-primary-intent, per-representative-completion) and treats per-cohort threshold compliance as a launch gate. Resolution rate per intent, handoff rate per intent, citation-coverage rate, regulatory-disclosure-compliance rate, intent-classification accuracy, cost-estimate accuracy versus actual claim adjudication for resolved estimates, and member-satisfaction all get sliced. A cohort with materially lower resolution rate after controlling for intent-mix factors is an equity issue that aggregate metrics hide. Launch is gated on every cohort meeting the threshold, not on the institution-wide average.
Multilingual deployment with validated plan-document translations. The demo is English-only. Most U.S. payer member populations include meaningful non-English-speaking groups, and many state Medicaid managed-care programs have specific language-access requirements. Per-language work: validated plan-document translations (with the translation reviewed by the compliance team for regulatory equivalency), per-language regulatory-disclosure phrasings, per-language tone calibration, per-language equity monitoring. Spanish-language deployment typically takes three to four additional months beyond the English go-live.
Voice-channel deployment for accessibility. The conversational logic above runs in chat. Adding a voice channel through Amazon Connect with Lex V2 reuses the orchestration logic but adds ASR and TTS layers, tighter latency budgets, voice-specific design (slower pacing, explicit member-ID confirmation, voice-friendly cost-estimate phrasing), and ASR error monitoring scoped to the benefits catalog. The voice channel makes the bot accessible to members without smartphones or with disabilities that make text input difficult.
Member-services routing with full case-context handoff. The demo's routing records a ticket. Production wires the routing to the institution's member-services queue (general member services, appeals-and-grievances team, behavioral-health benefits team, financial-counseling team, crisis pathway), with CTI integration with the call center and ticketing-system integration for asynchronous handoff. The handoff payload includes the conversation transcript, the retrieved evidence, the tool-call results, the intent classification, and the unresolved issue. The receiving agent picks up where the bot left off, not from scratch. The SLA, escalation procedure, and tabletop-drill cadence are documented per routing target.
Compensation operations for incorrect or disputed answers. When a member disputes an answer ("you told me this was covered, but it was denied"), the operations team needs operational tooling to reproduce the conversation, retrieve the cited evidence, confirm the bot was right or wrong, and either escalate the underlying coverage decision or compensate the member and feed the failure mode into the improvement loop. The compensation path is explicit, audited, and exercised in tabletop drills.
Disaster-recovery and degraded-mode operation. When upstream dependencies fail (Bedrock outage, eligibility unreachable, claims unreachable, formulary unreachable), the bot must degrade gracefully. The minimum behavior is "I'm having trouble pulling that data right now; would you like me to have someone from member services call you back?" Better is graceful warm handoff to live agents with the conversation context preserved. Document the per-mode behavior, test the failure modes in staging, and exercise the failover paths quarterly. Cached recent eligibility responses can serve answers with explicit "as-of" disclaimers when the live system is down.
Patient-rights workflow for conversation logs and decision records. Conversation logs are dense PHI plus financial information. Decision records are member communications. Members have rights to access both. The institution has retention obligations that vary by state and by record class. Build the workflow: how a member requests their benefits-conversation history and decision records, how the requests are authenticated, how the data is produced, how deletion requests interact with retention obligations, and how the decision records are referenced from the member portal for the member's own access.
Continuous-improvement loop with structured failure-mode labeling. Production transcripts surface intents the team did not define, retrieval gaps in the plan-document corpus, regulatory-disclosure gaps in the library, cost-estimate accuracy issues, parity issues for behavioral-health questions, citation gaps, and patterns in the decision-record journal that point to operational issues. Build the labeling and review workflow: review production transcripts weekly with compliance and benefits-administration leadership, propose plan-document-corpus updates, propose disclosure-library updates, propose cost-share-rule updates, propose prompt changes, run them through the evaluation set, deploy via versioned aliases, monitor for regressions. The bot's quality at month six is determined by whether someone is doing this work, not by how good the launch was.
Testing. There are no tests in this demo. A production pipeline has unit tests for the input-screening logic, the intent-classification logic (each category fires correctly, ambiguous queries trigger clarification), the per-question handlers (each tool is invoked with the right arguments, citations are attached correctly, regulatory disclosures are added when applicable), the cost-estimate-compute tool (deductible-then-coinsurance arithmetic, embedded-vs-aggregate family logic, separate-medical-and-pharmacy-accumulator handling), the citation-grounding verifier (every coverage assertion traces to a retrieved chunk; every cost number traces to the cost-estimate tool), the plan-version-stamp consistency check, the regulatory-disclosure-presence check, the output-screening replacement logic. Integration tests against a Bedrock test environment, non-production eligibility, claims, accumulator, UM, formulary, and provider-network endpoints with synthetic data, and a non-production plan-document corpus. End-to-end tests that simulate full conversations through representative scenarios including the surprise-bill case, the parity case, the cost-estimate case, and the multi-claim explanation case. Never use real PHI or PFI in test fixtures.
Observability beyond metrics. The demo emits CloudWatch metrics but no traces and no structured logs ready for cross-call investigation. Production runs CloudWatch Logs Insights queries that join across the API Gateway logs, the chat-handler logs, the screening logs, the Bedrock invocation traces, the tool-Lambda logs, the decision-record journal, and the audit records by session_id. AWS X-Ray traces show the latency contribution of each step. When a single conversation goes wrong, the on-call engineer needs to reconstruct the full trace in seconds, not minutes.
Cost monitoring and per-conversation attribution. Bedrock's per-token charges, Knowledge Bases' per-query charges, the vector store's hosting charges, and the per-call costs of the upstream-system integrations add up. Some benefits conversations are dramatically more expensive than others (a cost-estimate question with multiple service-code lookups and a follow-up coverage question costs more than a one-shot deductible-balance question). The cost-per-intent and cost-per-resolved-conversation analytics let the operations team see which intents are economically efficient and which warrant tooling improvements.
