Recipe 11.4: Python Implementation Example
Heads up: This is a deliberately simple, illustrative implementation of the pseudocode walkthrough from Recipe 11.4. It shows one way you could translate the pre-visit-intake-bot pipeline into working Python using boto3 against Amazon Bedrock (LLM with function-calling), Amazon Bedrock Knowledge Bases (managed RAG over the per-visit-type protocol-language phrasings, persona-and-tone guidance, sensitive-item phrasings, and screener-introduction templates), Amazon Bedrock Guardrails, Amazon Comprehend Medical (for clinical-entity extraction supplementing the LLM's HPI and ROS extraction), AWS Lambda, Amazon API Gateway, Amazon DynamoDB, Amazon S3, and Amazon EventBridge. The demo uses a

It is not production-ready. There is no real Bedrock Agents action group configured, no real Knowledge Base ingestion, no real Guardrail configuration, no API Gateway plumbing, no WAF rule tuning, no per-Lambda IAM least privilege, no KMS customer-managed keys, no VPC endpoints to the EHR, no Object-Lock-protected packet journal, no Connect contact-center handoff, no real validated-screener-translation pipeline, and no Secrets Manager wiring for the EHR credentials. Think of it as the sketchpad version: useful for understanding the shape of an adaptive conversational-intake AI pipeline that respects the input-screening discipline, the per-visit-type protocol-as-code discipline, the validated-screener-administration discipline, the structured-clinical-data-extraction discipline, the parallel-crisis-and-acuity-flagging discipline, the pre-visit-packet-handoff discipline, and the audit-everything discipline this recipe demands. It is not something you would point at a hospital website on Monday morning. Consider it a starting point, not a destination.The code maps to the ten pseudocode steps from the main recipe: receive the message, bootstrap or resume the session with greeting and disclosure, run input safety screening with intake-specific crisis sensitivity (Step 1); load encounter context, chart context, prior-intake context, and select the per-visit-type protocol (Step 2); drive the conversation through the question-flow state machine asking one question per turn and capturing the answer (Step 3); run the per-question extraction tool to convert the patient's free-text answer into structured findings (Step 4); run the crisis-and-acuity flagging pipeline in parallel against every utterance and finding (Step 5); handle a crisis interruption with explicit response templates and explicit routing pathways (Step 6); administer screeners with their validated wordings and item-by-item capture (Step 7); assemble and deliver the pre-visit packet to the EHR with a closing summary (Step 8); run the output safety screening with intake-specific scope-violation, hallucination, and persona checks (Step 9); persist the durable conversation record, the tool-call ledger, the partial-state cleanup, and the per-cohort metrics (Step 10). The synthetic patients, encounters, chart contexts, screener responses, and acuity flags in the demo are fictional; nothing in this file should be interpreted as advice from any real institution.
Setup
You will need the AWS SDK for Python:

In production you would also configure an Amazon Bedrock Agent (or a custom Lambda-based orchestrator) with action groups defining the intake tools (encounter_context_lookup, chart_context_lookup, prior_intake_lookup, protocol_selector, question_flow_state_machine, hpi_extraction, ros_extraction, medication_reconciliation_capture, allergy_reconciliation_capture, history_extraction, screener_administer, crisis_and_acuity_flagging, packet_assemble, packet_deliver), each backed by a tool-implementation Lambda that wraps the institution's EHR FHIR API, the institution's protocol registry, and the institution's screener registry. You would also configure an Amazon Bedrock Knowledge Base ingesting curated content from S3 covering the per-visit-type protocol-language phrasings, the institution's persona and voice guidance, the practice's preferred phrasings for sensitive items (mental-health, substance-use, intimate-partner-violence), the screener-introduction templates, and the closing-summary templates. You would configure an Amazon Bedrock Guardrail with restricted-topic filters for clinical-advice, diagnostic-speculation, treatment-recommendation, and severity-assessment categories at minimum, an API Gateway endpoint fronting the chat-handler Lambda, an AWS WAF web ACL with rate limits tuned for the resume-across-multiple-sessions intake pattern, the five DynamoDB tables (conversation-state, conversation-metadata, tool-call-ledger, intake-partial-state, flag-events), an Amazon S3 bucket with Object Lock in compliance mode for the pre-visit-packet journal sized to the longest of HIPAA's six-year minimum, the state's medical-records-retention rules, and the institutional regulatory floor, an EventBridge bus for intake-lifecycle events (conversation_started, intake_completed, intake_abandoned, acuity_flag_raised, crisis_flag_raised, packet_delivered, packet_delivery_failed, conversation_closed), a Kinesis Data Firehose delivery stream for the conversation-audit archive, AWS Secrets Manager secrets for the EHR credentials, and (where applicable) the Connect contact-center integration for the live-clinician handoff path on crisis. The demo replaces all of these with small mocks so the focus stays on the per-turn classification, identity-handling, protocol-state-machine, structured-extraction, screener-administration, crisis-flagging, and packet-delivery logic rather than on the platform plumbing.
Your environment needs credentials configured (via environment variables, an instance profile, or ~/.aws/credentials). The IAM role or user needs:

Scope each Lambda's IAM role to the specific resource ARNs it touches. The tutorial-level permissions above are fine for learning and will fail any serious IAM review. The chat-handler Lambda has Bedrock invocation and read-write on the conversation tables, but no direct access to the EHR. The chart-context-lookup Lambda has read-only access to Patient, Encounter, Condition, MedicationRequest, AllergyIntolerance, and Observation. The packet-deliver Lambda has the specific permission to write QuestionnaireResponse (or the institution's equivalent) to the encounter context; it does not have permission to modify the chart's medication, allergy, or problem lists. Separation of concerns by Lambda role limits the blast radius of any single Lambda's compromise. Avoid wildcard actions and resources in production.
A few things worth knowing upfront:
- The intake protocol per visit type is the bot. The code below assumes the practice's per-visit-type protocols are curated, dated, and version-controlled, with each visit type's chief-complaint-and-HPI scope, ROS scope, history scope, screener bundle, and packet schema explicitly encoded. Cleaning up the protocols is the project. Skip the cleanup and the bot asks generic questions for every visit type, producing a barely-better-than-form experience; do the cleanup and the bot conducts visit-type-appropriate intake that meaningfully changes the visit. The
PROTOCOL_LIBRARYplaceholder in the config is where you wire in your real protocols (in production these are encoded as versioned policy artifacts reviewed by clinical leadership and the relevant clinical service lines, not as a Python dict). - Validated screeners are not paraphrased. The PHQ-9, GAD-7, AUDIT-C have specific item wordings derived from validation studies; modifying the wordings invalidates the score. The screener tool encapsulates the validated wordings and the validated scoring rules. The LLM does not paraphrase screener items. The demo's
SCREENER_LIBRARYfollows the validated PHQ-2 and PHQ-9 wordings and scoring rules exactly. - Crisis-and-acuity flagging runs in parallel with the conversation. Every patient utterance and every captured finding runs through the crisis-and-acuity pipeline, regardless of where in the protocol the conversation is. Crisis flags interrupt the conversation and route to an explicit pathway. Acuity flags accumulate into the pre-visit packet and route to clinical staff post-conversation. The demo enforces the parallel pipeline as a separate component, not a feature of the conversational LLM.
- The bot does not modify the chart. Medication-reconciliation deltas, allergy-reconciliation deltas, and history updates are captured as patient-reported events. The clinical team confirms or rejects them during the visit and the chart change is a clinical action that happens at or after the visit. The demo's reconciliation tools produce structured deltas without touching the underlying chart.
- The pre-visit packet is the bot's value-delivery mechanism. The structured packet (chief complaint, HPI, ROS, reconciliation deltas, history updates, screener scores, acuity flags, new-information events, conversation transcript, version stamps) is what the clinical team consumes before the visit. If the packet does not land in front of the clinician at the right moment, the bot's value is largely lost. The demo's packet-deliver tool writes a FHIR-shaped QuestionnaireResponse plus a journal record; production wires this into the EHR's pre-visit-display configuration with clinical-leadership sign-off on the visual design.
- Resumability is part of the architecture. Patients fill out intake on lunch breaks, on phones with low batteries, in distracted environments. The conversation state, the protocol position, and the accumulated findings persist after each captured turn. Resume is graceful: the bot greets, summarizes what has been captured, asks if the patient wants to continue or restart. The demo's
intake_partial_statetable holds the resumable position; production has TTL tuned per visit type (typically 48-72 hours before the scheduled visit). - Conversation logs are dense PHI and a clinical record. The intake conversation contains chief complaint, HPI, ROS, medications, allergies, family history, social history, and screener responses. The log is HIPAA-relevant and many institutions treat it as part of the formal medical record. Audit logging, encryption, access controls, and retention policies apply. The demo writes a redacted record; production writes through Firehose into an Object-Lock S3 bucket sized to the longest of HIPAA's six-year minimum, the state's medical-records-retention rules, and the institutional regulatory floor.
- The output check verifies chart-fact references against tool results. A bot that says "I see you're allergic to penicillin" when the chart-context tool did not return a penicillin allergy is a hallucination with clinical-record-quality consequences. The demo's
screen_outputchecks every chart-fact reference in the response against the tool-call ledger; production extends this with stronger LLM-based grounding checks. - DynamoDB rejects Python
float. Every confidence score, threshold, time-window, screener score, and numeric metadata field passes throughDecimalon its way in and on its way out. The_to_decimalhelper handles it. - The example collapses many Lambdas into a single Python file. In production the chat handler, the input-screening function, the identity-handling function, each tool-implementation function, the crisis-and-acuity pipeline, the output-screening function, and the audit-archival function are separate Lambdas with their own IAM roles, error handling, retries, and DLQs. Comments call out where the boundaries should fall.
Configuration and Constants
Everything that is configuration rather than logic lives here. Resource names, model IDs, the per-visit-type protocol library, the validated-screener library, the acuity-pattern library, the persona templates, and the validation thresholds are what you would change between environments.

This demo runs end-to-end and produces the right disposition records, but the distance between it and a real intake bot serving a hospital patient population is significant. Here is where that distance lives.
Real Bedrock Agents action groups (or a custom LLM-and-tool orchestrator). The demo runs a fixed Python flow that bypasses the LLM's tool-calling abilities entirely; the conversation flow is hard-coded in the _conduct_intake_turn chain. Production wires the intake tools as Bedrock Agents action groups (one OpenAPI spec per tool with the argument schema, the response schema, and the docstring the LLM uses to decide when to call it), gives the agent the institutional persona prompt and the conversational-template Knowledge Base, and lets the LLM drive the turn-by-turn phrasing and the structured-extraction tool calls. The Python flow above is helpful for understanding what tools exist and what each one does; the production system lets the LLM decide the question phrasing and the extraction-tool arguments while the state machine and the schema validators keep the structure honest.
Real Bedrock Knowledge Base ingestion of the conversational-template corpus. The demo's phrased questions are hard-coded; production has a Knowledge Base ingesting curated content from S3 covering each per-visit-type protocol's question-language phrasings, the institution's persona and voice guidance, the practice's preferred phrasings for sensitive items (mental-health, substance-use, intimate-partner-violence), the screener-introduction templates, and the closing-summary templates. Real Bedrock Guardrails configuration. The demo passes GUARDRAIL_ID but does not actually configure a Guardrail. Production configures restricted-topic filters for clinical-advice, diagnostic-speculation, treatment-recommendation, and severity-assessment categories at minimum, plus Bedrock Guardrails' contextual-grounding feature for the response generation steps. The Guardrail is pinned to a specific version, tested against a held-out evaluation set, and updated on a versioned-rollout cadence with canary traffic.
Real per-visit-type protocols as code with clinical-leadership ownership. The demo's PROTOCOL_LIBRARY covers two visit types with starter question flows. Failure to detect a crisis in retrospective review is a high-severity incident with a structured root-cause analysis.
Real DynamoDB and S3 wiring. The mocks are dictionaries; production is DynamoDB tables with on-demand or provisioned capacity, customer-managed KMS keys, point-in-time recovery on the metadata, ledger, partial-state, and flag-events tables, TTL on the partial-state table tuned per visit type (typically 48-72 hours before the scheduled visit), and DynamoDB Streams emitting change events for downstream consumers. The pre-visit-packet-journal S3 bucket has SSE-KMS, Object Lock in compliance mode, lifecycle to S3 Glacier Instant Retrieval after 30 days and Glacier Deep Archive after 90, and retention sized to the longer of HIPAA's six-year minimum, the state's medical-records-retention rules, the state-specific consumer-privacy-law retention rules where applicable, and the institutional regulatory floor. The audit archive has its own KMS key separate from the packet-journal KMS key for blast-radius containment.
KMS customer-managed keys per data class. Every PHI-bearing resource (the five DynamoDB tables, the packet-journal bucket, the audit-archive bucket, the Firehose delivery stream, the Secrets Manager secrets, Lambda environment variables, CloudWatch Logs) uses customer-managed KMS keys with key rotation enabled. Different KMS keys for different data classes (conversation-state vs packet-journal vs flag-events vs audit-archive) limit the blast radius of any single key compromise. Key access is scoped to the specific principals that need it; CloudTrail logs every key-usage event.
VPC and VPC endpoints. The chat-handler Lambda runs internet-facing through API Gateway, which is the public design. The tool Lambdas that call the institution's EHR or registries run in a VPC with PrivateLink (where supported) or a tightly-scoped NAT-gateway path with allow-list. VPC endpoints for Bedrock, DynamoDB, S3, KMS, Secrets Manager, EventBridge, Comprehend Medical, HealthLake (if used), and CloudWatch Logs keep AWS-internal traffic off the public internet.
WAF tuning for the resume-across-multiple-sessions intake pattern. Intake endpoints have moderate rate limits because legitimate patients sometimes complete intake in a long single session and sometimes resume across multiple sessions; the limits accommodate the legitimate pattern while screening abuse. WAF rules also apply bot-detection rules that allow legitimate accessibility tools while blocking automated abuse, plus geo-restrictions if applicable, plus managed rule groups for common attack patterns.
Per-Lambda IAM least privilege with separation of concerns. The demo uses a single set of mocked credentials. Production has one IAM role per Lambda (chat handler, input screening, identity handling, each tool implementation, output screening, audit archival), each scoped to the specific resource ARNs the Lambda touches. The chart-context-lookup Lambda has read-only access to Patient, Encounter, Condition, MedicationRequest, AllergyIntolerance, and Observation. The packet-deliver Lambda has the specific permission to write QuestionnaireResponse but does not have permission to modify the chart's medication, allergy, or problem lists. The screener-administer Lambda has access to the screener registry. The protocol-selector Lambda has access to the protocol registry. Separation of concerns by Lambda role limits the blast radius of any single Lambda's compromise.
Per-cohort accuracy and equity monitoring with launch gates. The demo emits CloudWatch metrics with per-channel and per-language dimensions, which is enough for per-category dashboards. Production stratifies by cohort axes the institution monitors (per-language, per-channel, per-age-cohort, per-visit-type, per-proxy-completion, per-new-patient) and treats per-cohort threshold compliance as a launch gate. Completion rate per visit type, abandonment-by-stage rate, time-to-completion, screener positivity rates per screener, acuity-flag rate, crisis-flag rate, mis-extraction rate, screener-administration-fidelity rate (validated-wording preservation), packet-delivery-success rate, EHR-side-clinician-acknowledgment rate, and patient-feedback distribution all get sliced. A cohort with materially lower completion rate after controlling for visit-type-relevant factors is an equity issue that aggregate metrics hide. Launch is gated on every cohort meeting the threshold, not on the institution-wide average.
Multilingual deployment with validated screener translations. The demo is English-only. Most U.S. healthcare patient populations include meaningful non-English-speaking groups. Per-language work: validated screener translations (PHQ-9, GAD-7, AUDIT-C have validated translations for many major languages; the institution uses validated translations rather than ad-hoc machine translation), per-language HPI and ROS extraction patterns, per-language acuity-pattern detection, per-language persona and tone calibration, per-language crisis-template translation reviewed by native-speaker behavioral-health clinicians, per-language launch gates in the metric pipeline. Spanish-language deployment typically takes three to four additional months beyond the English go-live.
Voice-channel deployment for accessibility. The conversational logic above runs in chat. Adding a voice channel through Amazon Connect with Lex V2 reuses the orchestration logic but adds ASR and TTS layers (recipe 10.5 patterns), tighter latency budgets, voice-specific design (slower pacing, explicit medication-name read-back, voice-friendly screener wordings), and ASR error monitoring scoped to the intake catalog. The voice channel makes the bot accessible to patients without smartphones or with disabilities that make text input difficult.
Resumability with proper TTL tuning. The demo uses a single 72-hour partial-state TTL. Production tunes the TTL per visit type based on when the encounter is scheduled (a same-day urgent visit's intake should not have a 72-hour TTL; an annual physical scheduled three weeks out probably should). The resume-rate metric is monitored, and the TTL configuration is a per-visit-type asset owned by the operations team.
Disaster-recovery and degraded-mode operation. When upstream dependencies fail (Bedrock outage, EHR unreachable, screener-registry unreachable), the bot must degrade gracefully. The minimum behavior is "we are having trouble right now; the visit is still scheduled, please call the office if you need to update anything before then." Better is graceful warm handoff to live agents with the conversation context preserved. Document the per-mode behavior, test the failure modes in staging, and exercise the failover paths quarterly.
Patient-rights workflow for conversation logs and packets. Conversation logs are dense PHI. Packets are clinical records. Patients have rights to access both. The institution has retention obligations that vary by state and by record class. Build the workflow: how a patient requests their intake history and packets, how the requests are authenticated, how the data is produced, how deletion requests interact with retention obligations, and how the packets are referenced from the patient portal for the patient's own access.
Compensation operations for incorrectly-flagged or incorrectly-routed conversations. When the bot raises a crisis or acuity flag that turns out to be a false positive (and the patient is left wondering why a clinician called them), the operations team needs operational tooling to communicate, document, and reconcile. When the bot misses a flag that should have been raised, the retrospective review feeds the pattern-library improvement loop. The compensation path is explicit, audited, and exercised in tabletop drills.
Continuous-improvement loop with structured failure-mode labeling. Production transcripts surface intents the team did not define, extraction bugs, protocol-branch edge cases the rules did not anticipate, persona issues that are too subtle to catch in pre-launch testing, screener-administration-fidelity gaps, acuity-pattern false positives and false negatives, crisis-handling correctness gaps, and patterns in the packet journal that point to operational issues. Build the labeling and review workflow: review production transcripts weekly with clinical leadership, propose protocol updates, propose pattern-library updates, propose prompt changes, run them through the evaluation set, deploy via versioned aliases, monitor for regressions. The bot's quality at month six is determined by whether someone is doing this work, not by how good the launch was.
Testing. There are no tests in this demo. A production pipeline has unit tests for the input-screening logic, the protocol-state-machine (each branch fires correctly, branch-opening and branch-closing transitions work, position advances correctly across resume), the per-question extraction tools (each schema validates, ask-clarification triggers when expected), the screener-administration logic (validated wordings preserved verbatim, scoring rules applied per validation studies, item-9 crisis detection works), the acuity-pattern detection (the demo's exertional-chest-pain-with-family-history pattern fires on the canonical Marisol scenario; pattern false-positive cases do not fire), the crisis-routing pathway (each crisis category routes to its reviewed template; the routing target is correct), the medication-list integrity check (response cannot mention medications outside the active list; chart-fact references resolve to actual chart entries), the output-screening replacement logic. Integration tests against a Bedrock test environment, a non-production EHR endpoint with synthetic encounters and patients, and a non-production protocol-and-screener registry. End-to-end tests that simulate full conversations through representative scenarios including the crisis path, the acuity-flag path, and the resume-after-partial-completion path. Never use real patient data in test fixtures.
Observability beyond metrics. The demo emits CloudWatch metrics but no traces and no structured logs ready for cross-call investigation. Production runs CloudWatch Logs Insights queries that join across the API Gateway logs, the chat-handler logs, the screening logs, the Bedrock invocation traces, the tool-Lambda logs, the flag-events records, the packet-journal records, and the audit records by session_id. AWS X-Ray traces show the latency contribution of each step. When a single conversation goes wrong, the on-call engineer needs to reconstruct the full trace in seconds, not minutes.
Cost monitoring and per-conversation attribution. Bedrock's per-token charges, Knowledge Bases' per-query charges, Comprehend Medical's per-call charges, the vector store's hosting charges, and the per-call EHR-integration charges add up. Some intake conversations are dramatically more expensive than others (a chest-pain primary-care followup with the cardiac branch open and ten HPI questions and a screener costs more than a routine annual-physical update). The cost-per-visit-type and cost-per-completed-intake analytics let the operations team see which visit types are economically efficient and which warrant protocol tuning. Build the dashboard.
