Recipe 11.3: Python Implementation Example

Heads up: This is a deliberately simple, illustrative implementation of the pseudocode walkthrough from Recipe 11.3. It shows one way you could translate the prescription-refill-bot pipeline into working Python using boto3 against Amazon Bedrock (LLM with function-calling), Amazon Bedrock Knowledge Bases (managed RAG over the medication-information corpus and patient-facing protocol phrasings), Amazon Bedrock Guardrails, Amazon Comprehend Medical (for medication entity extraction with RxNorm coding), AWS Lambda, Amazon API Gateway, Amazon DynamoDB, Amazon S3, and Amazon EventBridge. The demo uses a MockBedrockRuntime standing in for LLM-driven intent classification and medication resolution, a MockEHR standing in for the institution's EHR (FHIR MedicationRequest, Observation, AllergyIntolerance, Condition resources), a MockEPrescribingPlatform standing in for the institution's Surescripts-routed e-prescribing setup, a MockKnowledgeBase standing in for the medication-information and protocol-language retrieval, a MockTable for each of the four DynamoDB tables (conversation-state, conversation-metadata, tool-call-ledger, co-signature-queue), a MockEventBus for EventBridge, a MockRefillJournal standing in for the S3 refill-event journal, and a MockCloudWatch for the metric emissions. It is not production-ready. There is no real Bedrock Agents action group configured, no real Knowledge Base ingestion, no real Guardrail configuration, no API Gateway plumbing, no WAF rule tuning, no per-Lambda IAM least privilege, no KMS customer-managed keys, no VPC endpoints to the EHR or e-prescribing network, no Object-Lock-protected refill-event journal, no Connect contact-center handoff, no real interaction-screening against the institutional CDS layer, and no Secrets Manager wiring for the EHR and e-prescribing credentials. Think of it as the sketchpad version: useful for understanding the shape of a transactional medication-management conversational AI pipeline that respects the input-screening discipline, the higher identity-verification floor refills demand, the medication-resolution-against-the-list discipline, the protocol-as-code discipline, the controlled-substance triple-defense discipline, the e-prescribe transactional discipline, the prescriber co-signature discipline, the refill-claim verification discipline, and the audit-everything discipline this recipe demands. It is not something you would point at a hospital website on Monday morning. Consider it a starting point, not a destination.

The code maps to the ten pseudocode steps from the main recipe: receive the message and bootstrap the session with greeting and disclosure plus input safety screening including refill-context crisis detection (Step 1), classify intent and route in-scope refill actions or hand off out-of-scope clinical questions and dose-change requests (Step 2), verify identity at the higher assurance floor refill actions require (Step 3), pull the patient's structured medication list and resolve the patient's free-text descriptor against it (Step 4), evaluate the practice's refill protocol against the resolved medication and chart context including lab reconciliation and interaction screening (Step 5), execute the disposition through the appropriate transactional tool with controlled-substance triple-defense (Step 6), handle status-check, cancel, and medication-question intents through their own paths (Step 7), handle e-prescribe transmission failures and partial-success cases without losing the patient's trust (Step 8), screen the output for scope drift and unsupported refill claims and medication-list-integrity violations (Step 9), and close the conversation, archive the durable audit record, and feed the refill-event journal (Step 10). The synthetic patients, medications, lab values, prescribers, pharmacies, and prescription IDs in the demo are fictional; nothing in this file should be interpreted as advice from any real institution.


Setup

You will need the AWS SDK for Python:

In production you would also configure an Amazon Bedrock Agent (or a custom Lambda-based orchestrator) with action groups defining the refill tools (patient_lookup, medication_list_lookup, medication_resolution, lab_reconciliation, interaction_screening, protocol_evaluate, e_prescribe, clinical_routing, refill_status_check, cancel_refill_request, cosignature_enqueue), each backed by a tool-implementation Lambda that wraps the institution's EHR FHIR API, the institution's clinical-decision-support layer (CDS Hooks where available), and the institution's e-prescribing platform (typically Surescripts-routed). You would also configure an Amazon Bedrock Knowledge Base ingesting curated content from S3 covering the medication-information corpus (each medication or class with its purpose, common questions, food-and-timing notes, side-effect summaries from the practice's preferred clinical reference), the patient-facing phrasings of the practice's refill protocol (so the bot can explain a routing decision in language the patient understands), and the practice's standing-order documentation (for staff-facing reasoning surfaces). You would configure an Amazon Bedrock Guardrail with restricted-topic filters for clinical-advice, dose-change-recommendation, medication-discontinuation-guidance, and controlled-substance-auto-approval categories, an API Gateway endpoint fronting the chat-handler Lambda, an AWS WAF web ACL with stricter rate limits on the refill endpoint than on either the FAQ or scheduling endpoints, the four DynamoDB tables (conversation-state, conversation-metadata, tool-call-ledger, cosignature-queue), an Amazon S3 bucket with Object Lock in compliance mode for the refill-event journal sized to the longest of HIPAA's six-year minimum, state-specific medical-records-retention rules, state-specific PDMP retention rules where applicable for controlled-substance-related records, state-specific pharmacy-record retention rules, state-specific consumer-privacy-law retention rules (CCPA/CPRA, VCDPA, CPA, etc.), per-channel retention obligations (TCPA/10DLC for SMS), and the institutional regulatory floor, an EventBridge bus for refill-lifecycle events (conversation_started, refill_requested, refill_auto_approved, refill_routed, refill_denied, refill_failed, cosignature_pending, cosignature_completed, controlled_substance_routed, crisis_detected, conversation_closed), a Kinesis Data Firehose delivery stream for the conversation-audit archive, AWS Secrets Manager secrets for the EHR and e-prescribing-platform credentials, and (where applicable) the Connect contact-center integration for the live-clinician handoff path. The demo replaces all of these with small mocks so the focus stays on the per-turn classification, identity-verification, medication-resolution, protocol-evaluation, e-prescribe-or-route, and disposition logic rather than on the platform plumbing.

Your environment needs credentials configured (via environment variables, an instance profile, or ~/.aws/credentials). The IAM role or user needs:

  • bedrock:InvokeModel for the intent-classifier model and for the orchestration model that decides tool calls and composes responses
  • bedrock-agent-runtime:InvokeAgent if you wire Bedrock Agents instead of running the orchestration in a custom Lambda
  • bedrock:Retrieve and bedrock:RetrieveAndGenerate on the Knowledge Base ARN that holds the medication-information corpus and the protocol-language phrasings
  • bedrock:ApplyGuardrail on the Guardrail ARN you configured for restricted-topic and harmful-content filtering
  • comprehendmedical:DetectEntitiesV2 and comprehendmedical:InferRxNorm for the medication entity extraction supplement
  • dynamodb:GetItem, dynamodb:PutItem, dynamodb:UpdateItem, dynamodb:Query on the conversation-state, conversation-metadata, tool-call-ledger, and cosignature-queue tables, scoped to the specific table ARNs
  • events:PutEvents on the refill-events bus for emitting refill-lifecycle events
  • firehose:PutRecord on the audit-archive Firehose delivery stream
  • s3:PutObject on the refill-event-journal bucket prefix
  • cloudwatch:PutMetricData for the operational metrics (auto-approval rate per medication class, time-to-completion, co-signature backlog, identity-verification success, tool-call success per tool, per-cohort slices)
  • secretsmanager:GetSecretValue on the EHR and e-prescribing-platform credential secrets pinned to the current rotation version
  • kms:Decrypt and kms:GenerateDataKey on the customer-managed keys protecting the conversation tables, the tool-call ledger, the cosignature queue, the refill-event journal, the audit archive, and the Secrets Manager secrets
  • For the tool Lambdas that call the institution's EHR, CDS layer, or e-prescribing platform: VPC-endpoint or PrivateLink permissions, plus whatever the EHR's and the e-prescribing platform's own auth flows require

Scope each Lambda's IAM role to the specific resource ARNs it touches. The tutorial-level permissions above are fine for learning and will fail any serious IAM review. The chat-handler Lambda has Bedrock invocation and read-write on the conversation tables, but no direct access to the EHR or e-prescribing platform. The medication-list-lookup Lambda has read-only access to MedicationRequest. The lab-reconciliation Lambda has read-only access to Observation. The protocol-evaluate Lambda has read-only access to chart context and the protocol artifact, with no e-prescribing permission whatsoever. The e-prescribe Lambda has the specific permission to invoke the e-prescribing platform but does not have permission to read the patient's full chart. Separation of concerns by Lambda role limits the blast radius of any single Lambda's compromise. Avoid wildcard actions and resources in production.

A few things worth knowing upfront:

  • The refill protocol is the bot. The code below assumes the practice's refill protocol is curated, dated, and version-controlled, with each medication class's auto-approval criteria, monitoring requirements, dosing rules, prescriber-authority rules, controlled-substance handling, and exceptions explicitly encoded. Cleaning up the protocol is the project. Skip the cleanup and the bot auto-approves things it should have routed and routes things it should have auto-approved; do the cleanup and the bot takes routine refills off the clinical staff's plate while preserving the prescriber's accountability. The REFILL_PROTOCOL placeholder in the config is where you wire in your real protocol (in production this is encoded as a versioned policy artifact reviewed by clinical leadership, not as a Python dict).
  • The medication list is the safety floor. The bot does not act on a medication unless the medication is on the patient's list. The medication-resolution tool returns a structured medication record from the list or returns "no match"; in the no-match case, the bot does not guess. This prevents the failure mode where the bot interprets "the white pill" as a medication that the patient does not take, runs the protocol against a wrong record, and produces a wrong action. The demo enforces this strictly.
  • Controlled substances do not auto-approve. Ever. Across every layer of the bot, every medication request is checked against the controlled-substance schedule. The protocol-evaluate tool returns controlled_substance_always_route for any controlled-substance request, the e-prescribe tool refuses to transmit controlled substances through the auto-approval path, and the output safety screening checks for controlled-substance language in the response. The triple defense is intentional. The demo follows the same pattern: the protocol path forces the safe routing disposition for any controlled substance regardless of what the LLM proposes.
  • Tool calls are the bot's contract with the EHR and the e-prescribing platform. Every action that affects the patient's medication record goes through a tool with a well-defined contract: arguments schema, response schema, error codes, idempotency semantics. The LLM proposes; the tools execute. The demo collapses each tool into a Python function for readability; production has each tool as its own Lambda with its own IAM role and its own retry semantics.
  • Lab reconciliation is part of the architecture, not an extension. The Eleanor failure mode (recent lab exists at an outside facility, has not yet been reconciled into the chart, protocol incorrectly finds "monitoring overdue") is so common that the bot's lab_reconciliation step runs before every protocol evaluation that needs monitoring data. The demo's lab_reconciliation_tool returns the most recent matching lab regardless of pending-reconciliation status.
  • Identity verification has a higher floor for refill actions. A patient logged into the patient portal asking to check on a pending refill needs lower assurance than an unauthenticated patient asking to e-prescribe a maintenance medication. Many institutions require authenticated portal sessions for any refill action that results in transmission. The demo's IDENTITY_POLICY table reflects this; production owns the policy as a versioned governance artifact reviewed by the privacy officer.
  • Prescriber co-signature is asynchronous but mandatory. Every auto-approved refill enqueues to the prescriber's co-signature queue with an SLA. The bot does not wait for the co-signature; the prescriber reviews within the institutional timeline (typically 24-72 hours). The demo's cosignature_queue table is the placeholder for what is normally a queue with SLA monitoring, escalation, and reporting wired into the prescriber's EHR inbox.
  • The refill-event journal is durable and separately governed. Conversation logs are PHI and have audit obligations; refill-event-journal records are clinical-record events and have medical-record-retention obligations. The demo writes both, but the production deployment has separate KMS keys, separate retention windows, and separate access controls for each.
  • Conversation logs are PHI by association. A patient interacting with the institution's refill bot has identified themselves as a patient of the institution and has discussed specific medications they take. The conversation log is HIPAA-relevant. Audit logging, encryption, access controls, and retention policies apply. The demo writes a redacted record; production writes through Firehose into an Object-Lock S3 bucket sized to the longest of HIPAA's six-year minimum, state-specific medical-records-retention rules, state-specific PDMP and pharmacy-record retention rules where applicable, state-specific consumer-privacy-law retention rules, per-channel retention obligations, and the institutional regulatory floor.
  • The output check verifies refill claims against tool results. A bot that says "your refill has been sent to Walgreens" when the e-prescribe tool did not actually return success is a bot that lets patients assume their medication is on the way when it is not, which can be a clinical-safety event. The demo's screen_output extracts refill-confirmation claims and verifies that each claim is supported by a successful e_prescribe tool result; production extends this with stronger LLM-based claim-vs-evidence checks.
  • DynamoDB rejects Python float. Every confidence score, threshold, time-window, lab value, and numeric metadata field passes through Decimal on its way in and on its way out. The _to_decimal helper handles it.
  • The example collapses many Lambdas into a single Python file. In production the chat handler, the input-screening function, the identity-verification function, each tool-implementation function, the output-screening function, and the audit-archival function are separate Lambdas with their own IAM roles, error handling, retries, and DLQs. Comments call out where the boundaries should fall.

Configuration and Constants

Everything that is configuration rather than logic lives here. Resource names, model IDs, the refill protocol, the identity-verification policy, the per-intent target assurance levels, the persona and refusal templates, and the validation thresholds are what you would change between environments.

The Gap Between This and Production

This demo runs end-to-end and produces the right disposition records, but the distance between it and a real refill bot serving a hospital patient population is significant. Here is where that distance lives.

Real Bedrock Agents action groups (or a custom LLM-and-tool orchestrator). The demo runs a fixed Python flow that bypasses the LLM's tool-calling abilities entirely; the conversation flow is hard-coded in the _resolve_medication, _evaluate_protocol, _execute_disposition chain. Production wires the refill tools as Bedrock Agents action groups (one OpenAPI spec per tool with the argument schema, the response schema, and the docstring the LLM uses to decide when to call it), gives the agent the institutional persona prompt and the medication-information Knowledge Base, and lets the LLM drive the multi-step orchestration. The Python flow above is helpful for understanding what tools exist and what each one does; the production system lets the LLM decide the order and the arguments. The tool-layer guardrails (controlled-substance refusal, medication-list integrity, protocol-as-code) remain in place no matter what the LLM proposes.

Real Bedrock Knowledge Base ingestion of the medication-information corpus and protocol-language phrasings. The demo's MockKnowledgeBase returns canned strings; production has a Knowledge Base ingesting curated content from S3 covering each medication or class with its purpose, common questions, food-and-timing notes, side-effect summaries, and the practice's patient-facing phrasings of refill-protocol decisions. The corpus has a named owner (the pharmacy team or the practice's clinical-content team), a documented review cadence (typically quarterly), and a versioned change-management workflow. The bot's quality on medication-question intents is bounded above by the corpus's quality.

Real Bedrock Guardrails configuration. The demo passes GUARDRAIL_ID but does not actually configure a Guardrail. Production configures restricted-topic filters for clinical-advice, dose-change-recommendation, medication-discontinuation-guidance, and controlled-substance-auto-approval categories, plus Bedrock Guardrails' contextual-grounding feature for the response generation steps. The Guardrail is pinned to a specific version, tested against a held-out evaluation set, and updated on a versioned-rollout cadence with canary traffic.

Real refill protocol as code with clinical-leadership ownership. The demo's REFILL_PROTOCOL dict is a tiny illustrative starter. Production has the protocol encoded as a versioned policy artifact owned by clinical leadership, reviewed by the medical-staff committee, signed by the participating prescribers as a standing-order delegation, sandbox-tested before each version goes live, staged-rollout with canary traffic, and version-stamped on every refill-event-journal record. The protocol covers each medication class's auto-approval criteria, monitoring requirements, dosing rules, prescriber-authority rules, controlled-substance handling, and exceptions. The protocol formalization typically takes three to six months of focused clinical-leadership work before the engineering project starts; skipping it is the most common reason refill-bot deployments fail in pilot.

Real EHR integration through a hardened tool wrapper. The demo's MockEHR is an in-memory dictionary; production has a hardened wrapper around the institution's actual EHR (FHIR MedicationRequest, Observation, AllergyIntolerance, Condition, and Patient resources for FHIR-capable EHRs; vendor-specific APIs for older systems; an integration-engine layer for institutions that route everything through Mirth, Rhapsody, or Cloverleaf). The wrapper handles every documented error code, retries idempotently with backoff, surfaces meaningful error categories to the bot rather than raw HTTP statuses, and is owned and maintained by the integration team. Plan multiple sprints for the integration; the LLM work is comparatively easy.

Real e-prescribing-platform integration with the institution's existing Surescripts setup. The demo's MockEPrescribingPlatform is a list and a counter; production wraps the institution's actual e-prescribing infrastructure with prescriber-authentication for transmissions (the bot acts under the prescriber's delegated authority but the transmission is properly attributed), pharmacy directory synchronization, transmission-error handling, retry policy, idempotency-key handling so a duplicate transmission is rejected at the platform, and Surescripts conformance verification.

Real CDS layer integration through CDS Hooks where available. The demo's MockCDS returns no interactions; production invokes the institution's clinical-decision-support layer (typically embedded in the EHR; CDS Hooks against the EHR's medication-prescribe hook where the EHR supports it). The integration includes interaction screening, contraindication checks, and policy enforcement. When the EHR exposes CDS Hooks, use the standard hook invocation; otherwise wrap the EHR's vendor-specific interaction-screening API.

Lab-reconciliation pipeline as an upstream prerequisite. The demo's lab_reconciliation_tool checks the in-memory lab list. Production depends on the institution's lab-reconciliation pipeline (recipe 5.6 patterns) reconciling outside-lab results into the chart in near-real-time. Without this upstream pipeline, the bot's route_with_monitoring_due rate is artificially high and the bot's value proposition is undermined. Investing in faster outside-lab reconciliation is part of the prerequisite stack.

Real medication resolution with the LLM and Comprehend Medical. The demo's medication_resolution_tool uses keyword overlap; production uses the LLM with the patient's medication list as context plus Amazon Comprehend Medical's RxNorm-coded medication entity extraction as a disambiguation supplement. Where the LLM's confidence is low and Comprehend Medical's extraction is unambiguous, the extraction wins; where they conflict, the bot asks the patient to clarify rather than guessing. Mis-resolved medications are tagged in the sampled-review queue and feed back into the prompt-tuning workflow.

Real DynamoDB and S3 wiring. The mocks are dictionaries; production is DynamoDB tables with on-demand or provisioned capacity, customer-managed KMS keys, point-in-time recovery on the metadata, ledger, and cosignature tables, TTL on the conversation-state table (idle sessions expire), and DynamoDB Streams emitting change events for downstream consumers (the cosignature-queue stream feeds the SLA-monitoring alarm). The refill-event-journal S3 bucket has SSE-KMS, Object Lock in compliance mode, lifecycle to S3 Glacier Instant Retrieval after 30 days and Glacier Deep Archive after 90, and retention sized to the longer of HIPAA's six-year minimum, the state's medical-records-retention rules, and the institutional regulatory floor. The audit archive has its own KMS key separate from the refill-event-journal KMS key for blast-radius containment.

KMS customer-managed keys per data class. Every PHI-bearing resource (the four DynamoDB tables, the refill-event-journal bucket, the audit archive bucket, the Firehose delivery stream, the Secrets Manager secrets, Lambda environment variables, CloudWatch Logs) uses customer-managed KMS keys with key rotation enabled. Different KMS keys for different data classes (conversation-state vs refill-event-journal vs audit-archive) limit the blast radius of any single key compromise. Key access is scoped to the specific principals that need it; CloudTrail logs every key-usage event.

VPC and VPC endpoints. The chat-handler Lambda runs internet-facing through API Gateway, which is the public design. The tool Lambdas that call the institution's EHR, CDS layer, or e-prescribing platform run in a VPC with PrivateLink (where supported) or a tightly-scoped NAT-gateway path with allow-list. VPC endpoints for Bedrock, DynamoDB, S3, KMS, Secrets Manager, EventBridge, Comprehend Medical, HealthLake (if used), and CloudWatch Logs keep AWS-internal traffic off the public internet. Endpoint policies pin access to the specific resources the bot uses.

WAF tuning with stricter rate limits on refill endpoints. Refill endpoints are abuse-prone in a way that FAQ and scheduling endpoints are not: a malicious actor attempting fraudulent refills under stolen identity has higher consequences than a spam booking. WAF rules apply stricter rate limits on refill endpoints, plus bot-detection rules that allow legitimate accessibility tools while blocking automated abuse, plus geo-restrictions if applicable, plus managed rule groups for common attack patterns.

Per-Lambda IAM least privilege with separation of concerns. The demo uses a single set of mocked credentials. Production has one IAM role per Lambda (chat handler, input screening, identity verification, each tool implementation, output screening, audit archival), each scoped to the specific resource ARNs the Lambda touches. The protocol-evaluate Lambda has read-only access to chart context with no e-prescribing permission whatsoever. The e-prescribe Lambda has the specific permission to invoke the e-prescribing platform but does not have permission to read the patient's full chart. The medication-list-lookup Lambda has read-only access to MedicationRequest. Separation of concerns by Lambda role limits the blast radius of any single Lambda's compromise.

Identity-verification policy as a versioned governance artifact. The demo's IDENTITY_POLICY is a Python dict; production stores the policy in Parameter Store or AppConfig (so it can be updated without redeploying the Lambda), reviewed by the privacy officer with a documented change-management workflow, version-stamped on every conversation's audit record so any reported issue can be reproduced against the policy state at the time. The Lambda reloads the policy at the start of each invocation.

Prescriber co-signature workflow with SLA monitoring. The demo enqueues to the cosignature-queue table. Production has the queue wired into the prescriber's EHR inbox (or a dedicated review interface) with SLA monitoring (percentage of auto-approvals co-signed within the SLA window), escalation (overdue items escalate to the supervising clinician), and reporting (weekly co-signature backlog dashboard reviewed by clinical leadership). When a prescriber flags a co-signature for retrospective clinical review, the flag feeds the protocol-improvement loop. CloudWatch alarms on the SLA-violation rate, with thresholds tuned to the institution's traffic.

Compensation operations for refilled-but-wrong medications. The demo auto-approves and routes but does not implement the operational tooling for compensating refills that turn out to be wrong (wrong medication identification, protocol applied to incomplete data, prescriber-flagged retrospectively). Production builds compensation operations: "view this refill's history," "reverse this refill with reason," "contact the patient and the pharmacy if the medication has not been picked up," all preserving the audit trail of the original and the compensation. Operations team owns the tooling; engineering builds it; compliance reviews the audit-trail completeness.

Per-cohort accuracy and equity monitoring with launch gates. The demo emits CloudWatch metrics with per-channel and per-language dimensions, which is enough for per-category dashboards. Production stratifies by cohort axes the institution monitors (per-language, per-channel, per-age-cohort, per-medication-class, per-authentication-path) and treats per-cohort threshold compliance as a launch gate. Auto-approval rate per medication class, time-to-completion, mis-resolved-medication rate, identity-verification success rate, prescriber-flagged-cosignature rate, tool-call success rate, handoff rate, and patient-feedback distribution all get sliced. A cohort with materially lower auto-approval rate after controlling for protocol-relevant clinical factors is an equity issue that aggregate metrics hide. Launch is gated on every cohort meeting the threshold, not on the institution-wide average.

Multilingual deployment. The demo is English-only. Most U.S. healthcare patient populations include meaningful non-English-speaking groups. Per-language work: native-speaker review of medication names (especially brand-name versus generic-name conventions, which vary by language), native-speaker review of the persona and refusal templates, per-language scope rules where culture-specific phrasings change the categorization, per-language identity-verification phrasings, per-language equity gates in the metric pipeline. Spanish-language deployment typically takes three to four additional months beyond the English go-live.

Voice-channel deployment for accessibility. The conversational logic above runs in chat. Adding a voice channel through Amazon Connect with Lex V2 reuses the orchestration logic but adds ASR and TTS layers (recipe 10.5 patterns), tighter latency budgets, voice-specific design (slower pacing, explicit medication-name read-back ("you said metformin, the diabetes medication, is that right?"), voice-friendly phrasings of medication names that are difficult to pronounce), and ASR error monitoring scoped to the medication catalog. The voice channel makes the bot accessible to patients without smartphones or with disabilities that make text input difficult.

Disaster-recovery and degraded-mode operation. When upstream dependencies fail (Bedrock outage, EHR unreachable, e-prescribing platform down, CDS layer slow), the bot must degrade gracefully. The minimum behavior is "we are having trouble right now, please call the office at the number." Better is a graceful warm handoff to live agents with the conversation context preserved. Document the per-mode behavior, test the failure modes in staging, and exercise the failover paths quarterly.

Patient-rights workflow for conversation logs and refill events. Conversation logs and refill-event-journal records are PHI by association and clinical-record events respectively. HIPAA grants patients the right to access their records. Build the workflow: how a patient requests their refill conversation history and refill events from the bot, how the institution authenticates the request, how the data is produced, how deletion requests interact with retention obligations.

Continuous-improvement loop with structured failure-mode labeling. Production transcripts surface intents the team did not define, medication-resolution bugs, identity-verification friction patterns, protocol-evaluation edge cases the rules did not anticipate, scope cases the rules did not anticipate, persona issues that are too subtle to catch in pre-launch testing, and patterns in the refill-event journal that point to operational issues. Build the labeling and review workflow: review production transcripts weekly with clinical leadership, propose protocol updates, propose prompt changes, run them through the evaluation set, deploy via versioned aliases, monitor for regressions. The bot's quality at month six is determined by whether someone is doing this work, not by how good the launch was.

Testing. There are no tests in this demo. A production pipeline has unit tests for the screening logic, the intent classifier, the medication-resolution tool against ambiguous and discontinued and specialist medications, the protocol-evaluate tool (controlled substances always route, monitoring-overdue cases route correctly, auto-approval cases match expected criteria, early-refill detection works), the controlled-substance triple-defense (the e-prescribe tool refuses transmission, the output check catches auto-approval language), the refill-claim verifier (every prescription-ID claim must match a ledger entry), the medication-list integrity check (response cannot mention medications outside the active list), the output-screening replacement logic. Integration tests against a Bedrock test environment, a non-production EHR endpoint with synthetic patients and labs, and a non-production e-prescribing endpoint. End-to-end tests that simulate full conversations through representative scenarios including the controlled-substance defense, the misuse-signal crisis path, and the lab-reconciliation-saves-the-refill case. Never use real patient data in test fixtures.

Observability beyond metrics. The demo emits CloudWatch metrics but no traces and no structured logs ready for cross-call investigation. Production runs CloudWatch Logs Insights queries that join across the API Gateway logs, the chat-handler logs, the screening logs, the Bedrock invocation traces, the tool-Lambda logs, the refill-event-journal records, and the audit records by session_id. AWS X-Ray traces show the latency contribution of each step. When a single conversation goes wrong, the on-call engineer needs to reconstruct the full trace in seconds, not minutes.

Cost monitoring and per-conversation attribution. Bedrock's per-token charges, Knowledge Bases' per-query charges, Comprehend Medical's per-call charges, the vector store's hosting charges, and the per-call EHR-integration charges add up. Some intents are dramatically cheaper than others (a check_refill_status lookup is much cheaper than a multi-turn request_refill conversation that goes through medication resolution, lab reconciliation, interaction screening, and protocol evaluation). The cost-per-intent and cost-per-resolved-conversation analytics let the operations team see which intents are economically efficient to handle in the bot and which are not. Build the dashboard.
